Luzerne County’s KAZ program is coordinated by three local Program Partners: The Greater Hazleton Civic Partnership, and Live Well Luzerne County and the Wilkes-Barre YMCA ACHIEVE initiative.  The KAZ Program Partners have built a coalition of more than 75 Funding, Media, Prize and Planning Partners working together to make active living the easy choice by connecting people of all ages and abilities to their local outdoor resources.


Greater Hazleton Civic Partnership improves quality of life in the Hazleton area.  One key project for the Civic Partnership is the Greater Hazleton Rails to Trails, a proposed 16 mile trail that will connect Hazleton and Eckley Miners Village to Lehigh Gorge State Park, and the D&L trail, a 165 mile corridor connecting North East Pennsylvania to Southeast Pennsylvania, following the path that anthracite coal took to market.  The D&L Trail is the core of the Delaware and Lehigh National Heritage Corridor.


Live Well Luzerne County is a county wide health promotion coalition which grew out of the work of Steps to a Healthier PA Luzerne County (Steps) and the Wyoming Valley Wellness Trails Partnership.  Steps, in operation from 2004 to 2009, was funded by the Centers for Disease Control through the PA Department of Health.  Luzerne County was one of  40 communities nationally to particpate in the Steps to a Healthier US Initiative ; the nation’s first program to fund an integrated approach to evidence-based chronic disease prevention and health promotion at a community level.  The Wyoming Valley Wellness Trails Partnership (WVWTP), in operation between 2003 and 2010, was one of 25 programs funded by the Robert Wood Johnson Foundation’s Active Living by Design Program focused on creating community led change by working with local partners to build a culture of active living.  Locally, the WVWTP worked to increase the use of local trails to improve health and encourage physical activity.   Live Well Luzerne County is a volunteer organization that promotes the use of proven program, policy, systems and environmental changes that create sustainable healthy, active communities.  LWL is housed at the Wilkes-Barre YMCA and manages the Wilkes-Barre YMCA’s Action Community for Health, Innovation and Environmental Change (ACHIEVE) innitiative.

The Wilkes-Barre YMCA’s Action Community for Health, Innovation and Environmental Change (ACHIEVE) initiative was launched in 2008 to support and advance community leadership as part of national efforts to prevent chronic diseases and related risk factors.  ACHIEVE is supported by the Centers for Disease Control and Prevention and is a partnership involving the National Association of Chronic Disease Directors, the National Association of County and City Health Officials, the National Recreation and Park Association and YMCA of the USA.  The purpose of ACHIEVE is to bring together local leaders and stakeholders to develop and implement policy, systems and environmental change strategies that can help prevent or manage health risk factors for obesity, heart disease, stroke, diabetes, cancer, arthritis and other chronic diseases.  Risk factors such as tobacco use and exposure, insufficient physical activity and poor nutrition contribute greatly to the development and severity of many chronic diseases.  The ACHIEVE model capitalizes on the experience and expertise of national organizations in strengthening community leadership, building capacity and activating change.  Establishing partnerships with national organizations to implement policy, systems and environmental changes is an efficient and effective mechanism for achieving chronic disease prevention and health promotion goals in communities.
